2013-14 Physical Form
Parkview Physicians Group will be conducting Sports Physicals at the Bryan Office
Tuesday, June 11 Male Athletes
Thursday, June 13 Female Athletes
Monday, June 17 Male Athletes
Wednesday, June 19 Female Athletes
Between 5:30 - 6:30 p.m. (first-come, first-served)
Cost: $10.00 (benefits EHS Athletic Department)
All athletes in grades 7-12 are required to have a physical. Each Athlete must bring an OHSAA Physical Form (see above) that is signed by their parent/guardian.
